If it hit within, say, a few hundred metres, it'd break through some if not all of the bone. A solid-y bullet, depending on what metal it was made with, will probably go all squishy and just tear up the muscles around the entry wound. If the metal is brittle, like a cheap alloy or from a less-than-awesome forge, it might shatter. God knows where it'd go then. Up the leg, or through the foot. maybe into the pelvic region.

If it just got the muscle, and it was cleaned properly (cauterised with fire, or cleaned and kept under a good bandage that was replaced every few hours), it'd heal with veeery little damage. However if it hit the bone, or wasn't kept clean, there'd be the issue of infection. They might lose the leg, since an unclean wound can cause gangrene or blood poisoning.

If no blood poisoning, and the bone was broken, only if it was splinted very well, and looked after with lotsa rest, would it heal okay-ish. Then there'd probably be a slight limp and arthritis issues for the rest of their life.

If no bone, just muscle, then movement of the leg would be an issue. But most (unless in a really tricky and unlucky spot) will heal relatively well given a few months of clean bandages, rest, special stretches to help the muscle recover.

If it hit near the foot or knee or hip, those joint bones and cartilage could be damaged both from the shot and from the misuse of joints due to pain or modified walking. The kneecap, depending on the age of the character (outside 20-40 = uber bad) might shatter or be knocked out of place, which would also modify their stance and walking pattern. Um... could be fucked for life.

But best case scenario, the bullet lands in the thigh and doesn't shatter, doesn't break bone, and the wounded receives proper burny-sanitisation care.

I'm sure they'd know how to use clean bandages, etc, but the availability of these might affect the situation. No coconut IV, in case of blood poisoning or loss, so that might be an issue.

If the leg ends up being amputated, they wouldn't know how to seal off the nerve endings, and pain and "I still have a leg" sensations would occur pretty frequently.

Hope that's useful. I don't know much about the muscular system; just what I picked up from Human Skeletal Analysis last year, and general forensic articles I've read on the side during that course.

Keep in mind that shotgun casings are much larger and devastating than others, such as pistols, and if shot close enough to hit bone would probably ruin the whole leg in this sort of world.
